About Irinotecan Amaksa

IRINOTECAN AMAKSA is an antineoplastic concentrate for infusion containing 20 mg of irinotecan hydrochloride trihydrate per 1 ml. Each vial contains 15 ml.

Composition

The active substance is irinotecan. Each millilitre contains 20 mg of irinotecan hydrochloride trihydrate (equivalent to 17.33 mg of irinotecan).

Excipients include water for injections, D-sorbitol, lactic acid and sodium hydroxide.

Indications

The medicine treats advanced colorectal cancer. With 5-fluorouracil and folinic acid it is used in chemotherapy-naive patients; as monotherapy when 5-fluorouracil has failed.

It is also combined with cetuximab in metastatic colorectal cancer with wild-type KRAS and increased EGFR expression, and in first-line regimens with 5-fluorouracil, folinic acid and bevacizumab, or with capecitabine with or without bevacizumab.

Pharmacological properties

Irinotecan is a semisynthetic camptothecin derivative and specific DNA topoisomerase I inhibitor. It is metabolised to SN-38, which is more cytotoxic to tumour cells, mainly via carboxylesterase in tissues.

It inhibits DNA topoisomerase I, causing single-strand DNA damage and blocking replication. The effect is S-phase specific and time-dependent. The medicine also inhibits acetylcholinesterase activity.
